A 47-year-old man arrives at the office from out-of-state. HIV positive for more than 10 years, he is heavily treatment experienced with many different regimens. His baseline labs show the following:

CD4+ cell count: 7 cells/mm3
Viral load: well over 100,000 copies/mL
Weight: 135 pounds (down from 170)
Some evidence of renal toxicity
Across-the-board resistance (including high levels to all PIs)
M184V mutation present
