Is there anything tamoxifen cannot do? Its activity in metastatic 
breast cancer, for which tumour-response rates may be about
40%, encouraged the use of tamoxifen at an earlier stage of disease. 
The drug is now a standard adjuvant treatment for women
with early breast cancer, but an overview published last year 
suggests that the scope and effectiveness of the drug as an adjuvant
have been underestimated. Used properly (that is, in women with 
tumours positive for oestrogen receptors [ER] and for at least 5
years), it is associated with reductions in the risk of recurrence 
of cancer approaching 50%. Moreover, the previously held notion
that tamoxifen is less effective in premenopausal than in postmenopausal 
women is wrong.
