Dear Mary,

Generally, my experience and understanding with any new treatment is
second line and used in conjuction with another treatment because it has
been proven that combinations of treatments appear to be more positive
in terms of response than if administered individually.  However in time
should this medicine prove to be more effective it then becomes first
line or protocal of treatment.  It needs to be proven first.  Hope this
helps understand clearer.  There may be other reasons as well.
