I just spent 8 days at Kaiser Permanente in San Francisco, and there was no
loudspeaker in the room at all. 
Mind you, it still was impossible to sleep from noise from machines and
people, but a loudspeaker would have
been the end.  I probably would have coerced my sister into breaking it...
