METHOTREXATE [MTX] 8 - 12 g/m2 IV
Maximum dose is 20 g
Caution: Monitor urine pH and MTX plasma concentrations carefully

LEUCOVORIN [Leu] 15 - 25 mg PO or I
q 6 hr for 10 doses, beginning 24 hr after MTX

CYCLE: wk x 2-4 wks
